CFM56-7B VSV Bellcrank Failure

We have experienced a VSV bellcrank failure on one of our engine (P/N:1276M84G15). You may find attached the photos of the damaged assembly. Fortunately this was not a IFSD. This came without an indication on engine trends. I would like to know if any of operators has seen such failure.
